I do not have any metallic chains on me. Light jewelry is acceptable.

I do not have an implanted electronic device including: pacemaker, defibrillator, cochlear hearing device, spinal stimulator, insulin injector, etc.

I do not have an organ transplant.

I do not have seizures or blood clots.

I am not actively bleeding, hemorrhaging, or having heavy menstruation.  Blood flow will increase with a PEMF treatment.

I understand if I choose to do a PEMF session during menstruation it may increase my flow, cramping etc.

I am not pregnant.

I have not received chemotherapy in the last 48 hours.

I agree to be fully responsible for any damages, if I do not follow the above listed consent agreement.
It is the responsibility of the patient to disclose any underlying physical condition or illness. PEMF Health Inc. will not provide treatment if we are made aware of adverse conditions as stated above.
I further understand Curtis Swischuk from PEMF Health Inc. is an Independent Certified Swiss Bionic Solutions Health Technician and NOT a licensed medical provider and he does not diagnose nor prescribe medications or supplements and his advice is not to be replaced with the advice of a licensed medical practitioner. 
I hereby request and consent to therapy to be performed by PEMF Health Inc., Curtis Swischuk, and his staff . I give permission and authority to provide therapy in ways that are judged beneficial to me. I feel adequately informed to do so and I accept all liability for my health and wellness choices using PEMF.

I understand that PEMF Health Inc. does not make any promises of cures of any kind and results may vary. I understand that results are not guaranteed.
